在参加属地实验室安全引入培训前，您需要与HSE老师/导师或主管沟通，了解HSE相关安全培训课程，并安排好您的学生、员工或自己参与相关课程，并通过HSE安全培训考试（简单流程见【附件1】）。 同时，您可以进行实验动物中央实验室 (LAF) 的相关线上课程并通过LAF的理论培训考试（详细流程见【附件2】）。

LAF属地实验室安全引入培训流程如下：
1. 大仪系统预约培训时间。
2. 培训前，请先行填写《Laboratory-Safety-Orientation-Form-LAF》 、《F-OHS-001-V1 医疗筛查调查问卷》和《F-PM-004-V1 知晓并遵守规章制度承诺书》（看附件）双面打印，每个文件3份，带到培训现场。
3. 现场填写《Laboratory-Safety-Orientation-Form-LAF》，属地安全员签名 。
4. 培训后，完整填写《Laboratory-Safety-Orientation-Form-LAF》内信息，本人签名、导师/主管签名后，将 1份《Laboratory-Safety-Orientation-Form-LAF》交回 LAF E4-415C办公室黄希文老师处。经审核后获得 LAF(GZ)准入资格。


Before attending the training for the introduction of laboratory safety in the territorial laboratory, you need to communicate with your HSE teacher/tutor or supervisor to understand the HSE related safety training courses and arrange for your students, staff or yourself to participate in the related courses and pass the HSE safety training exam (see [Annex 1] for a simple process). At the same time, you can conduct Laboratory Animal Central Laboratory (LAF) online courses and pass the LAF theory training exam (see [Annex 2] for the detailed process).

The training process for the introduction of laboratory safety in LAF territories is as follows:
1. Reserve a training time in the Da Yi system.
2. Before the training, please fill out the Laboratory-Safety-Orientation-Form-LAF
F-OHS-001-V1 Medical Screening Questionnaire and F-PM-004-V1 Commitment to Know and Comply with Rules and Regulations (see attachments). Print both sides of the documents, make 3 copies of each and bring them to the training site.
3. Complete the Laboratory-Safety-Orientation-Form-LAF on-site and have it signed by the local safety officer. The Laboratory-Safety-Orientation-Form-LAF is completed on site and signed by the territorial safety officer.
4. after training, complete the information on the Laboratory-Safety-Orientation-Form-LAF in its entirety, sign for yourself, sign for your instructor/supervisor, and return 1 copy of the Laboratory-Safety-Orientation-Form-LAF to LAF E4-415C office to Ms. Huang Xiwen. LAF(GZ) admission will be granted after review and approval.
